Massachusetts Health Quality Partners (MHQP), your health plan, and its participating doctors and hospitals are committed to providing outstanding care and services. To help them and to provide all Massachusetts residents with information to support decisions about their health care, Massachusetts Health Quality Partners (MHQP) annually reaches out to patients to learn about their experiences with their providers. MHQP is an independent measurement and reporting group dedicated to improving patient care.

• Your answers to the survey will remain confidential to protect your privacy.
• Your answers to the survey will be combined with those from hundreds of other patients. This means that no one will know any one person's answers to the survey.
